Inverse Psoriasis: Symptoms, Treatment and Gentle Care for Skin Folds

Psoriasis in skin folds

Inverse psoriasis is psoriasis that appears in skin folds, such as the armpits, groin, under the breasts, around the navel and between the buttocks. Because these areas are warm, moist and rubbed, the patches look smooth, shiny and red or dark rather than thick and scaly. Fold skin is thin, so treatment favours gentle, steroid-sparing options and good daily care to reduce friction and sweat.

Key points
  • Looks like: smooth, sharply bordered red, purple or dark patches with little scale, often sore or itchy.
  • Often mistaken for intertrigo, yeast or fungal infection.
  • Steroid-free tacrolimus and pimecrolimus cleared or almost cleared fold psoriasis in 65% to 71% of trial patients.
  • Daily care: keep folds dry, reduce friction, wear breathable fabrics and use gentle, steroid-free soothing creams.

What does inverse psoriasis look like?

Inverse (flexural or intertriginous) psoriasis forms smooth, shiny, well-defined patches. On lighter skin they look bright red; on darker skin they can look purple, brown or grey. Moisture stops the thick scale of plaque psoriasis from building up, and the skin often feels sore, raw or itchy, especially with sweat and friction (Micali et al., 2019).

  • Armpits
  • Groin and genital skin
  • Under the breasts and abdominal folds
  • Between the buttocks
  • Behind the knees and ears, and around the navel

Many people also have classic plaques elsewhere. For the full picture, see our psoriasis guide.

How is inverse psoriasis different from intertrigo or a fungal rash?

Condition Typical clues
Inverse psoriasis Sharp borders, shiny surface, psoriasis elsewhere or in the family, nail pitting
Intertrigo Raw, damp redness from friction and sweat, often mirrored on both skin surfaces
Candida (yeast) Bright red with small "satellite" spots beyond the edge
Tinea (ringworm) Scaly, raised, spreading edge with a clearer centre

Because these overlap and can occur together, a doctor may take a skin scraping before treating. Read more on intertrigo and ringworm.

How is inverse psoriasis treated?

Fold skin is thin and occluded, so it absorbs creams more and is more prone to steroid side effects such as thinning and stretch marks (Barnes et al., 2015). A systematic review found evidence for topical immunomodulators, vitamin D analogues and short courses of topical steroids, with biologics for severe disease (Reynolds et al., 2020).

  • Tacrolimus 0.1% ointment: 65.2% of patients were clear or almost clear after 8 weeks vs 31.5% with placebo (Lebwohl et al., 2004).
  • Pimecrolimus 1% cream: 71% were clear or almost clear by week 8 (Gribetz et al., 2004).
  • Mild topical steroids: short courses for flares, as your doctor directs.
  • Vitamin D analogues such as calcipotriol, often alternated with other treatments; newer steroid-free creams may also be options (Hong et al., 2021).

How should you care for psoriasis in skin folds?

  1. Keep folds clean and dry. Wash gently with a mild, fragrance-free cleanser and pat dry, including between folds.
  2. Reduce friction. Wear loose, breathable cotton or moisture-wicking fabrics.
  3. Manage sweat. Change out of damp clothes quickly after exercise.
  4. Use a thin barrier layer where skin rubs.
  5. Avoid fragranced products and harsh scrubs.
  6. Weight management can reduce fold depth and friction where relevant.

Steroid-free care for skin folds. QICAOGANGMU pairs Ku Shen and She Chuang Zi with cooling borneol and menthol, tested steroid-free by TÜV SÜD PSB. Rated 4.5 to 4.7 out of 5 on Amazon. See the formula

Can steroid-free herbal creams help?

Steroid-free creams are useful for daily comfort in folds, where long-term steroid use is limited. In laboratory studies, Ku Shen compounds reduced psoriasis-like inflammation (Lin et al., 2024) and osthole from She Chuang Zi blocked the TRPV3 itch channel (Sun et al., 2018). Cooling ingredients that act on TRPM8, like menthol, reduced itch in a real-life study (Misery et al., 2019).

On fold skin, apply only a thin layer to intact skin, patch test first, and avoid mucous membranes and genital skin. If it stings more than briefly, wash it off. For more herb detail, see herbal remedies for psoriasis.

When should you see a doctor?

  • A fold rash that is not clearly diagnosed, or does not improve in 2 weeks
  • Cracks, weeping, pain or signs of infection
  • Genital involvement, or psoriasis affecting intimacy or daily life
  • Joint pain or swelling
