Baby Eczema vs Diaper Rash: How to Tell Them Apart and Soothe Your Baby's Skin
Baby skin care
Baby eczema usually appears as dry, itchy, red patches on the cheeks, scalp, arms and legs, and often spares the diaper area. Diaper rash appears where the diaper sits and is caused by wetness, urine and stool. Both improve with gentle care: lukewarm baths, fragrance-free products, thick moisturisers for eczema, and a zinc or petrolatum barrier cream with frequent changes for diaper rash.
- Eczema favours the face and limb creases; diaper rash stays in the diaper area.
- Thick, fragrance-free moisturisers are the foundation of baby eczema care.
- A bright red diaper rash with small red spots at its edge may be yeast and needs an antifungal.
- Creams with menthol or camphor-like ingredients are not recommended for babies under 2.

Is it baby eczema or diaper rash?
| Baby eczema | Diaper rash | |
|---|---|---|
| Where | Cheeks, scalp, outer arms and legs; later elbow and knee creases | Buttocks, genitals, upper thighs |
| Looks like | Dry, rough, red patches; may ooze when flaring | Red, shiny skin where the diaper touches |
| Itch | Very itchy; baby rubs or scratches | Sore, especially when wet |
| Cause | Weak skin barrier, family history of allergies | Wetness, urine, stool, friction |
How do you care for baby eczema?
- Bathe for 5 to 10 minutes in lukewarm water with a fragrance-free, soap-free wash.
- Pat dry and apply a thick, fragrance-free moisturiser or ointment within 3 minutes.
- Moisturise at least twice a day, even when the skin looks clear. Regular moisturising reduced eczema flares across 77 trials (van Zuuren et al., 2017).
- Dress your baby in soft cotton and keep nails short.
- Use fragrance-free laundry products and avoid overheating.
- Ask your doctor about prescription treatment if eczema flares despite good care.
How do you care for diaper rash?
- Change diapers often, and straight away when soiled.
- Clean gently with water or fragrance-free wipes; pat dry.
- Give some diaper-free time each day.
- Apply a thick layer of zinc oxide or petrolatum barrier cream at each change.
- Fasten diapers loosely so air can circulate.
What does a yeast diaper rash look like?
If a diaper rash lasts more than 2 to 3 days despite good care, is bright red with raised edges, reaches into the skin creases and has small red spots beyond its edge, it may be a Candida yeast infection. This needs an antifungal cream, so see your doctor or pharmacist.
Which creams are suitable for babies?
| Age | Suitable products |
|---|---|
| Under 2 years | Plain fragrance-free moisturisers, petrolatum and zinc barrier creams; anything else on a doctor's advice |
| 2 years and older | As above, plus gentle steroid-free soothing creams following the label, after a patch test |
