Perianal itching (pruritus ani) is itching of the skin around the anus. It is usually caused by moisture, irritation from wiping, soaps or wet wipes, or a skin condition such as eczema, and less often by hemorrhoids, infection or pinworms. Most cases settle with gentle cleaning, keeping the area dry, avoiding irritants and a soothing barrier or herbal cream.
- Pruritus ani is a symptom with many possible causes, so finding and removing the trigger matters most.
- Over-cleaning and fragranced or preserved wet wipes are common, often overlooked triggers.
- Cooling ingredients such as menthol are approved in the US for external anal itch at 0.1% to 1%.

What is pruritus ani?
Pruritus ani is persistent itching around the anus. Doctors describe two types: secondary pruritus ani, where a specific cause such as dermatitis, hemorrhoids or infection is found, and primary (idiopathic) pruritus ani, where no single cause is identified and irritation and scratching keep the itch going (Jakubauskas et al., 2023). Itching is often worse at night, after bowel movements or in hot weather.
What causes itching around the anus?

German guidelines on perianal dermatitis recommend identifying and removing irritants and contact allergens, and patch testing when allergy is suspected (Weyandt et al., 2020). The preservative methylisothiazolinone in wet wipes is a recognised cause of perianal allergic dermatitis (Admani et al., 2014).
How do you stop anal itching at home?
- Clean gently with water. Rinse after bowel movements with plain water or a mild, fragrance-free cleanser. Avoid scrubbing.
- Pat completely dry. Use soft toilet paper or a cool hairdryer setting. Moisture keeps the itch going.
- Stop using scented or preserved wet wipes, bubble baths and perfumed soaps.
- Wear loose cotton underwear and change it daily, or after sweating.
- Do not scratch. Scratching damages the skin and makes the itch worse. Keep nails short and consider cotton gloves at night.
- Keep stools soft and regular with fibre and fluids, which reduces straining and leakage.
- Protect the skin with a thin layer of a bland barrier ointment or a soothing cream.

How does Traditional Chinese Medicine treat anal itching?
In TCM, persistent itch in the anal and genital area is linked with Damp-Heat pouring downwards, often combined with Wind that causes itch. Treatment aims to clear Heat, dry Dampness and stop itching, traditionally with external washes and creams. She Chuang Zi and Ku Shen are a classic external pair for damp, itchy skin in this region.
Modern research supports the itch-relieving potential of these ingredients.
