Ringworm Treatment: Antifungal Creams, Natural Remedies and Chinese Herbal Research

Fungal skin guide

Ringworm (tinea) is a common fungal skin infection, not a worm. Most cases on the body, groin and feet clear with an antifungal cream such as terbinafine or clotrimazole used for 2 to 4 weeks. Natural options with research include tea tree oil and Chinese herbs such as She Chuang Zi. Avoid steroid creams on a suspected ringworm rash, because they can make it spread.

Key points
  • In a Cochrane review, people using terbinafine cream were about 4.5 times more likely to be clinically cured than with placebo.
  • In a randomised trial, 25% to 50% tea tree oil gave a marked response in about 70% of people with athlete's foot, compared with 39% on placebo.
  • She Chuang Zi extract inhibited Trichophyton rubrum, the commonest ringworm fungus, in laboratory tests.
  • Scalp and nail ringworm usually need prescription tablets.

What is ringworm?

Ringworm is a skin infection caused by dermatophyte fungi, most often Trichophyton rubrum. It usually appears as an itchy, red, scaly ring with a raised edge and clearer centre that slowly spreads outwards. Fungal skin infections affect an estimated 10% to 20% of people worldwide (El-Gohary et al., 2014). It spreads through skin contact, shared towels, gym floors and infected pets (CDC).

Ringworm has a scaly, raised edge and a clearer centre; nummular eczema is evenly inflamed; psoriasis has thick silvery scale.

What are the types of ringworm?

Body site Name Usual treatment
Body Tinea corporis Antifungal cream for 2 to 4 weeks
Groin Tinea cruris (jock itch) Antifungal cream; keep the area dry
Feet Tinea pedis (athlete's foot) Antifungal cream; dry feet and change socks daily
Scalp Tinea capitis Prescription antifungal tablets
Nails Onychomycosis Prescription tablets or medicated nail lacquer

What is the best treatment for ringworm?

Over-the-counter antifungal creams clear most ringworm on the body, groin and feet. A Cochrane review of 129 trials found people using terbinafine cream were about 4.5 times more likely to be clinically cured than with placebo, and clotrimazole 1% and naftifine also worked well (El-Gohary et al., 2014).

  1. Apply the antifungal to the rash and about 2 cm of surrounding skin, usually once or twice a day.
  2. Continue for 2 to 4 weeks, and for a week or two after the rash has cleared, as directed on the pack.
  3. See a doctor if it is not improving after 2 weeks, is spreading, or affects the scalp or nails.

Resistant strains of ringworm are increasingly reported, so a doctor may take a scraping to confirm the fungus (Caplan et al., 2025; CDC).

Why should you avoid steroid creams on ringworm?

Steroid creams calm redness and itch but do not kill the fungus. On ringworm they can let the infection spread while hiding its typical ring shape, a problem called tinea incognito. The CDC advises against using steroid creams on a rash that could be ringworm (CDC). This is one reason to choose steroid-free products for itchy rashes whose cause is uncertain.

What natural remedies work for ringworm?

Tea tree oil

In a randomised, placebo-controlled trial of 158 people with athlete's foot, 25% and 50% tea tree oil solutions gave a marked clinical response in 72% and 68% of users, compared with 39% on placebo (Satchell et al., 2002). Tea tree oil can irritate or cause allergy in some people, so test it first. See tea tree oil on skin.

Chinese herbs

She Chuang Zi (Cnidium monnieri) extract inhibited Trichophyton rubrum in laboratory tests and damaged the fungus's structure (Yanyun et al., 2021). Its main compound, osthole, also showed antifungal activity against Candida yeast (Li et al., 2018). Read more in our She Chuang Zi research guide.

Keeping skin dry

Fungi thrive in warm, damp skin. Drying thoroughly after washing, wearing breathable fabrics and changing socks daily make every treatment work better.

How does Traditional Chinese Medicine treat ringworm?

In TCM, fungal skin rashes are called xuan (癣) and are linked with Dampness, Heat and Wind. The traditional principle is to dry Dampness, clear Heat, "kill parasites" and stop itching, usually with external washes and creams. She Chuang Zi is traditionally used externally to dry Dampness, kill parasites and stop itching, including for tinea-type rashes, and is commonly combined with Ku Shen (She Chuang Zi materia medica; Ku Shen materia medica).

Where QICAOGANGMU fits

QICAOGANGMU is a steroid-free Chinese herbal cream built on the classic external pair for damp, itchy skin: She Chuang Zi 3%, the herb with laboratory activity against Trichophyton rubrum, and Ku Shen 1.5%. Cooling Bing Pian (borneol) 2% and menthol 0.5% soothe itch, and Bai Bu 0.5%, traditionally used to "kill parasites", completes the formula. Because it is steroid-free, it avoids the risk of tinea incognito, and finished batches are independently tested by TÜV SÜD PSB, with Not Detected results for eight corticosteroids.

For a confirmed fungal infection, use an antifungal treatment for the full course. Test QICAOGANGMU on a small area first and follow the packaging directions.

How do you stop ringworm coming back?

  • Keep skin clean and dry, especially between the toes and in skin folds.
  • Change socks and underwear daily; wear breathable cotton.
  • Wear sandals in gym showers and pool areas.
  • Do not share towels, clothes, hairbrushes or bedding.
  • Wash hands after touching the rash or a pet with patchy hair loss, and have the pet checked by a vet.
  • Finish the full antifungal course even if the rash looks better.

Frequently asked questions

What kills ringworm fastest?

An antifungal cream such as terbinafine, used once or twice a day, is the fastest proven option for ringworm on the body, groin or feet. Many cases improve within 1 to 2 weeks, but keep using it for the full course.

Can ringworm be treated naturally?

Tea tree oil has trial evidence for athlete's foot, and Chinese herbs such as She Chuang Zi have laboratory antifungal activity. Keeping the skin dry helps every treatment. If the rash is not clearing, use a proven antifungal.

Is ringworm contagious?

Yes. It spreads through skin contact, shared towels and clothing, damp floors and infected pets. It becomes much less contagious once treatment has started.

Can I use a steroid cream on ringworm?

Not on its own. Steroid creams can make ringworm spread and harder to recognise. Use an antifungal, and choose steroid-free products for itchy rashes of uncertain cause.

How long does ringworm take to go away?

Body ringworm usually clears in 2 to 4 weeks with an antifungal cream. Scalp and nail infections take longer and usually need prescription tablets.
